New patients may be seen at our outreach clinic in St. Cloud, MN. Appointments can be
made for Tuesdays afternoons.
Be sure to have the following information available before calling:
• Name( s), address( es), phone number( s) of patient and family member( s)
• Patient birth date
• Insurance plan information including identification number( s), policy holder's
name and employer's name
• Social security number (If a U.S. resident)
• Name, address and phone number of your family physician and the physician
who is treating your seizures
Referral From Physicians
We prefer to work closely with your family doctor and welcome referrals. However, it is not necessary to obtain a physician's referral to make an appointment at MINCEP
unless it is a requirement of your health insurance plan. If your insurance plan requires
that you have referral, our office must have the referral on record before you can be
seen.
